FILL THIS FORM OUT FIRST (AND COMPLETE REGISTRATION PROCESS) BEFORE REGISTERING FOR ANY EVENTS

You will receive an onscreen message, and email, providing direct links to nominate for some, or all of the 2015 CAVS events.  Please complete your registration form first, as this will assist you with the nomination process.
